Welcome to “Paying it Forward,” a monthly interview series made possible by Piedmont Health. In this series, we hear from the folks at Piedmont about the importance of community health centers – and why they chose a career in community health.

This month, Aaron welcomes Yvette Moore, team director of the PACE program at Piedmont Health’s location in Burlington.
“I’m originally from New York City,” she says. “We relocated here in 2024, (but) I’ve been in healthcare for roughly about 34 years. I’ve worked for some of the larger health systems, I’ve spent some time in home care and hospice, and now I’ve kind of transitioned over into community health and working with PACE…
“I’m enjoying it a lot. I’m learning a lot. I love the fact that we’re able to serve vulnerable populations. I think it’s great work.”
That work involves helping older adults age in place, providing them with needed care without requiring them to move into assisted living facilities or nursing homes.
“PACE stands for Program of All-inclusive Care for the Elderly,” Moore says. “What makes us unique is that it brings together medical care, therapy, medications, transportation, social services, and daily support under one coordinated team… (and) the participants are able to socially engage with others. There’s meals provided, there’s a lot of activities. So it’s great. And if they have medical needs or (if they’re) experiencing a medical issue, they’re able to get that in real time.”
As the team director, Moore is not only responsible for her patients – she’s also responsible for her staff, in an extremely demanding field where frequent turnover is not uncommon. Luckily, with over three decades of experience in health care, she knows how to make sure her staff feel just as supported as her patients.
“Maintaining morale requires intentional leadership,” she says. “For me, I focus on three things: connection, recognition, and purpose. Teams need an opportunity to communicate openly, support one another, and feel that their work matters. So we celebrate wins, even the small ones…. Reminding staff that they are helping people remain in their homes and communities provides a powerful sense of meaning and sustains resilience.”
